Insurance reimbursement process if you live in California

Lief Rx is currently covered by the following insurance plans in CA:

  • Beacon

  • Optum

  • United Healthcare

If your insurance plan works with Lief Rx based on the above list, please fill out our eligibility screener to see if one of our providers can authorize your Lief Rx prescription.

If you complete the screener and provide your correct and complete insurance information at the end, we’ll be able to submit your claim for reimbursement to your insurance provider at the end of each month.

If you have not met your deductible, you will be responsible for covering the full cost of the program (max cost of $255 per month depending on your utilization of services) until you meet your deductible, at which point your costs will be covered by your insurance plan. If you owe a co-pay, that amount will be charged. Processing times vary from payer to payer, ranging from 14 to 60 days.

Please keep in mind that in order for your Lief device rental costs to be covered, you need to have worn your device at least 16 days per calendar month. If you do not adhere to the program by wearing your device for the 16 day minimum, it will be your responsibility to cover your Lief device rental cost out-of-pocket.

Insurance reimbursement process if you do not live in California

In order to get Lief covered by insurance, call your insurance provider and ask if your plan covers the following CPT codes for remote patient monitoring: 99453, 99454, & 99457.

If covered, a physician needs to determine your eligibility for Lief in order for your insurance to reimburse you. Please fill out the following form with your physician’s (primary care provider, psychiatrist, clinical psychologist, etc.) information so we can contact them on your behalf.

Cost of Lief Rx if you’re a telemedicine patient using insurance

We believe in price transparency so that if you have in-network insurance with Lief, you can understand what we’ll be billing your insurer, as well as prevent surprise bills.

If you have met your deductible, your plan should cover the full cost of services that you use. If you have not met your deductible, you will be responsible for paying for the Lief services you utilize until you meet that deductible. See the table below for a breakdown of those costs:

We highly recommend calling your health plan to check if the above services are covered under your plan (you can share the CPT codes listed above). At that time you can also gain a more specific understanding of any of your patient responsibility (e.g., deductible, co-pays, co-insurance, etc.).

If you’re uninsured or your health insurance plan is out-of-network, this information does not apply to you. Read the next section for more details.

Cost of Lief without insurance

If you are not using insurance, you can still access Lief as a consumer (vs. as a patient). The cost of the Lief program will be either $69/month or $99/month depending on the subscription plan you choose on our order page.

The key difference is that the $99 plan includes 1-on-1 coaching with a highly trained Lief coach, who will communicate with you each week by Zoom, phone, and/or chat to help you understand your HRV data, help you learn the self-regulation technique of HRV biofeedback, and ultimately help you achieve your health and wellness goals.

You may be able to use your HSA or FSA account to pay for your monthly Lief subscription. We recommend checking with your HSA benefits administrator to confirm your coverage.
